Examinations at our Office

EX1-X (Executive Examination)

First Examination two and a half hour, includes:

• Complete Medical History Evaluation,
• Complete Physical Examination, (no internal is required)
• Complete Diet Evaluation with Dr. Srajeldin’s Diet Book,
• Urine Dipstick Test, Blood Typing, Blood Sugar,
• Blood Work that includes:

1. Blood allergy test of 200 food allergens test plus 40 inhalants allergens test
2. Cholesterol Panel, or Equivalent test
3. Liver Enzymes, or Equivalent test
4. CBC, or Equivalent test
5. Creatinine , or Equivalent test



EX1-A (Inclusive Examination)

First Examination 150 minutes, includes:

• Complete Medical History Evaluation,
• Complete Physical Examination,
• Diet Evaluation with Dr. Srajeldin’s Diet Book,
• Urine Dipstick Test, Blood Typing, Blood Sugar,
• Blood Work Includes:
  1. Cholesterol Panel, or Equivalent test
  2. Liver Enzymes, or Equivalent test
  3. CBC, or Equivalent test
  4. Creatinine , or Equivalent test
  5. Laboratory Blood Allergy test is NOT included

 

EX1-B (Full Examination)

First Examination 150 minutes, includes:

• Complete Medical History Evaluation,
• Complete Physical Examination,
• Diet Evaluation, with Dr. Srajeldin’s Diet Book,
• Urine Dipstick Test, Blood Typing, Blood Sugar and Blood Cholesterol
• Laboratory blood work and blood allergy tests are NOT included

 

EX1-S (Single Symptom Examination)

First Examination 60 minutes, includes:

• Medical History Evaluation of Symptom being sought for treatment,
• Physical Examination of the condition being treated,
• Urine Dipstick Test, Blood Typing, Blood Sugar and Blood Cholesterol.
• No blood laboratory work
• No diet set up
• Laboratory blood work, allergy tests and Diet setting are NOT included

EX1-C Child (Children Examination up to age 15)

First Examination 150 minutes, includes:

• Complete Medical History Evaluation,
• Complete Physical Examination,
• Diet Evaluation,
• Urine Dipstick Test, Blood Typing, Blood Sugar and Blood Cholesterol.
• Laboratory blood work and allergy tests are NOT included

 

EX-2 (Alternate Examination)

Subsequent Examination 120 minutes (for those who have been away from the office for more than two years), includes:

• Re-Complete Medical History Evaluation,
• Complete Physical Examination,
• Re-Diet Evaluation,
• Urine Dipstick Test and Blood Sugar.
• Laboratory blood work and allergy tests are NOT included

EX-3 (Subsequent Examination)
Subsequent Examination 90 minutes (for those who have been away from the office for more than one year), includes:

• Re-Complete Medical History,
• Physical Examination,
• Re-Diet Evaluation,
• Urine Dipstick Test and Blood Sugar
• Laboratory blood work and allergy tests are NOT included

 

EX-5 (Progress Check Examination)

Subsequent Examination 30 minutes (Current Patients) , includes:

• Re-Evaluation of last visit’s Symptoms,
• Re-examination of last visit’s symptoms,
• Check for any aggravations,
• Check for new symptoms,
• Adjust Remedies and diet according to improvement,
• Laboratory blood work and allergy tests are NOT included
